Process for Getting Connected with Accessibility Services

Step 1

Provide our office with your contact information and documentation of your diagnosis:

  • Individualized Education Plan (IEP) or 504 Plan
  • A letter from your provider that contains:
  • Letter head
  • Providers signature
  • Formal diagnosis

The documentation can be scanned/emailed, faxed, or dropped off in person to our office.

Step 2

Schedule an appointment with the coordinator to complete an intake

  • Coordinator will contact you once your documentation and contact information have been received
  • Intakes are typically around 45 minutes to an hour long

Complete the intake with coordinator!
