Process for Getting Connected with Accessibility Services

Step 1:

Set up an appointment with our Accessibility Services Director and/or Provide our office with your contact information and documentation of your diagnosis (documentation is not required for initial meeting):

  • Individualized Education Plan (IEP) or 504 Plan
  • A letter from your provider that contains your diagnosis and provider’s credentials.

The documentation can be scanned/emailed, faxed, or dropped off in person to our office.

Step2:

If you have submitted documentation our director will reach out to you to schedule the intake. Intake take 30-60 minutes in length and can be in person or via zoom.

Step 3:

Complete intake with Director of Accessibility Services to access your accommodations.
