Forms Official Transcript Request Form (pdf) The Records and Registration office maintains student academic records. Transcript records show all course-work for which a student was registered during each term of enrollment and the grades awarded for those courses. Requests for transcripts can be made in person, by fax, or by mail. Transcript requests will not be accepted via e-mail. ATTENTION: A $5.00 fee is charged for each official transcript requested. A check or money order (no cash) made payable to Central Lakes College must accompany the request. Visa or Master Card are also accepted. There is no fee for unofficial transcripts, which are also available on the Student E-Services site. Students will need their Student Tech ID and PIN number to access their unofficial transcripts on the E-Services site.

Application for Graduation (pdf) It is preferred that students fill out the Application for Graduation form before the start of the last term they plan to attend. When a student completes the Application for Graduation form and returns it to the Records and Registration office they will be notified:

A course may be repeated for an improved grade. Only the highest grade earned will be counted toward a degree and in the computation of the overall numerical grade point average. This policy applies to all grades including “F” grades. However, both the original and the repeated grade will appear on the student’s transcript. Tuition and fees will be charged each time. Since repeated coursework is not automatically updated on a student’s record, a student who is repeating a course needs to fill out a Course Repeat form. Name Address or SSN Change Form (pdf)
After the first five days of the semester until ten days before the first day of final exams, a student may withdraw from a class. The student will receive a “W” grade for the class and will still be responsible for 100% of the tuition and fees for the withdrawn class. Withdrawing from a class will not affect the student’s GPA. However, it could affect his/her probation/suspension status. If you choose to withdraw from all of your classes, please visit with the Financial Aid office for a Total Withdraw form. If you are concerned about your probation/suspension status please visit with a counselor BEFORE withdrawing.
