This page is your source of forms to be used for financial aid purposes.  Some of the links are instructions on how to do something and others are forms that may be required to complete your financial aid file. 

Your "Aid Application Status" in your E-Services account will detail the form(s) required to complete your CLC Financial Aid file.  Please select the forms you need to complete from the list provided at the links below.  Be sure to choose the correct link for the correct academic year.

Please Note: For your privacy and protection, it is the policy of CLC's Financial Aid Office to shred any documentation or form received in our office that is not required for your file at that time.

The following list includes forms or process instructions that are not academic year specific, but can be used in any academic year.

Authorization for Release of Information Form:  Federal Regulations do not allow CLC to talk to anyone about your financial aid information unless you have signed a release giving CLC permission.   This form will allow you to give CLC that permission.

Consortium Agreement:   A consortium agreement is a tool that allows a student to receive aid from one college while counting the credits at each college being attended to determine the credit level at which to pay financial aid.   It needs to be completed each term if you want to count credits at more than one college.  You are responsible to pay tuition at each college, even if you do complete a consortium.

If you are receiving financial aid from CLC, enrolled in a CLC program and would like to take credits at another college while you are taking credits at CLC, the CLC Financial Aid Office has a consortium for you to fill out.

MNSCU college and CLC consortium  complete if the other college you are attending is a MNSCU college.  (MNSCU college list is attached to this consortium)
NON-MNSCU and CLC consortium  complete if the other college you are attending is not a MNSCU college.
 
FAFSA Signature correction: If your "Aid Application Status"  explained that your file is rejected for signatures, please go to the www.fafsa.gov website to correct. 

IRS Data Retrieval Tool:  This process allows you and/or your parent(s) to download filed Federal tax return data into your FAFSA.

IRS Tax Return Transcript:  This process demonstrates how to request a transcript of your and/or your parent"s Federal tax return to be used by CLC for the verification process.
