2010-11 Financial Aid Forms (Fall 2010, Spring 2011 and Summer 2011)
    
  Financial Aid forms to complete your file for the 2010-11 school year are not available yet.  Watch for a financial aid status report in the mail or online at Student E-Services.  Computer set up is still in the works and will hopefully be ready to go by the end of March 2010.

Post-Secondary Child Care Waiting List:  If you may have child care expenses for the 2010-11 school year you should get on the application waiting list for the Post-Secondary Child Care grant.  You need to be on the waiting list prior to June 1, 2010 for priority consideration.  Some time in June applications will be sent by email or US postal mail to those who are on this list.  Click on the link below to complete information required for the waiting list.

Below is a list of forms, which includes the necessary one(s) to finish your financial aid file and package an award letter. Please refer to the "Financial Aid Status Report" available online at Student E-Services or the paper one mailed to you to determine the forms you need to complete.  Also, make sure you are completing forms for the correct school year.

To submit your forms electronically:

1.  Click on the link that begins with the "www.dynamic"  for a form you need to complete.
2.  Follow the instructions to login.  IF you have not used Dynamic forms before, please be sure to "Create an Account" before logging in.  If you need to complete the Dependent Verification Worksheet your parent will need to "Create an Account" also, to sign electronically.

How to create an account
a. Click on “Create an account”
b. Use your own email address when creating your account (Do not use your parent's or a sibling's email account)
c. Write down your Username and Password
d. Click on “Submit”
e. You will receive a "Thank you" and click on "Login to Complete Form"
f.  Now "Sign In" using the Username and Password you just created and click "Submit"

3.  At the “Welcome” screen, click on “Continue to Form”
4.  Complete and submit form
5.  You will then receive a confirmation page that will say "Thank you!" and have the option of printing your form for your records
6.  Repeat if you have another form to submit

To submit your forms by paper:
1.  Click on the pdf format for a form you need to complete  
2.  Print 
3.  Complete the form 
4.  Mail the form to Central Lakes College, Financial Aid Office, 501 West College Drive, Brainerd MN  56401

Please Note: For your privacy and protection, it is the policy of CLC's Financial Aid Office to shred any documentation or form received in our office that is not required for your file at that time.

Loan Request Form (pdf)
If you already have a loan for 2009-10 at CLC, this form can be used to request additional loan funds if you have remaining eligibility.  Please contact the loan officer to verify whether or not you have any remaining eligibility.

Post-Secondary Child Care grant application: If you will be attending CLC the fall of 2009 and will have child care expenses the application will be available very soon.

Special Circumstances Appeal form:  This form allows special situations to be considered with regard to changes in income, large medical and/or dental expenses that have been paid out of pocket, along with other possibilities

Consortium Agreement: Students may take classes at more than one school in any given term. Although, students should not receive aid from both schools. A consortium agreement works as a tool to allow a student to receive aid at one school, but be able to count credits at each school to determine the credit level at which to pay financial aid.

If you are receiving financial aid from CLC, enrolled in a CLC program and would like to take credits at another college while you are taking credits at CLC, the CLC Financial Aid Office has a consortium for you to fill out. The consortium needs to be completed each term if you have credits at another college. Even if you complete a consortium with CLC it is your responsibility to pay the tuition at the other school.
