Forms

Below is a list of forms, which includes the necessary one(s) to finish your financial aid file and package an award letter. Please refer to the "Financial Aid Status Report" available online at Student E-Services or the paper one mailed to you to determine the forms you need to complete. 

Also, make sure you are completing forms for the correct school year.

To submit and sign your forms electronically:

1.  Click on the link that begins with the "www.dynamic"  for a form you need to complete.
2.  Follow the instructions to login.  
3.  IF you have not used Dynamic forms before, please be sure to follow the steps of "Are you a new user?" to create a username and password.  If you need to complete the Dependent Verification Worksheet your parent will also need a username and password, to sign electronically.

     How to get a Username and password:
        a. Click on “Get Started”
        b. Use your own email address when creating your account (Do not use your parent's or a sibling's email account)
        c. Write down your Username and Password
        d. Click on “Create Account”
        e. You will receive a "Thank you" and click on "Login to Complete Form"
        f.  Now "Sign In" using the Username and Password you just created and click "Log In"

4.  At the “Welcome” screen, click on “Complete This form”
5.  Complete and submit form
6.  Follow the directions for signature requirements
7.  You will then receive a confirmation page that will say "Thank you!" and have the option of printing your form for your records
8.  Repeat if you have another form to submit

To submit your forms by paper:
1.  Click on the pdf format for a form you need to complete  
2.  Print 
3.  Complete the form 
4.  Mail the form to Central Lakes College, Financial Aid Office, 501 West College Drive, Brainerd MN  56401

Please Note: For your privacy and protection, it is the policy of CLC's Financial Aid Office to shred any documentation or form received in our office that is not required for your file at that time.

Post-Secondary Child Care Application:  Please print and complete this application if you will have child care expenses for the school year.  Awarding of this grant does not begin until September 1, 2010 or later.

OTHER FORMS

2010-11 Decline Work or Request additional loan funds:  This form is to be used when a student wants to decline the work award on the 2010-11 Official Award letter or ask for additional loan funds when a previous request has been made.  Please print the pdf document, complete and sign it.  Then mail or bring the form into the Financial Aid Office.  Click on this link for the form, "2010-11 Decline Work or Request additional loan funds".

Special Circumstances Appeal:  The financial aid office in certain situations can adjust information on a FAFSA due to "Special Circumstances".  Please use the following list to determine if you may qualify for the "Special Circumstances Appeal".
      Have you or your familly experienced any of the following:
      1.  A large drop in income for 2010 compared to 2009?
      2.  Death or illness in your family resuslting in a large drop in income?
      3.  Divorce or separation since completing the FAFSA?
      4.  Unusual medical or dental expenses that you have had to pay out of pocket?
      5.  Parent, who is enrolled in college at lease half-time seeking a degree?
      6.  Other unusually large out of pocket expenses related to your attendance at Central Lakes College. 
           ie. tools,  very expensive books, required computer or software, travel of more than 260 miles a week, etc.

If so, please complete the "Special Circumstances Appeal" form, print it off, sign it and submit all requested documentation detailed on the form to the Financial Aid Office.
Please Note:  If you have a Federal Pell Grant award in the amount of $2775 per term on your award letter, you do not qualify for a special circumstances appeal.  You already have the maximum amount of grant eligibility.

 Consortium Agreement: Students may take classes at more than one school in any given term. Although, students should not receive aid from both schools. A consortium agreement works as a tool to allow a student to receive aid at one school, but be able to count credits at each school to determine the credit level at which to pay financial aid.

If you are receiving financial aid from CLC, enrolled in a CLC program and would like to take credits at another college while you are taking credits at CLC, the CLC Financial Aid Office has a consortium for you to fill out. The consortium needs to be completed each term if you have credits at another college. Even if you complete a consortium with CLC it is your responsibility to pay the tuition at the other school.

Consortium agreements can be confusing. Becky Davis is the contact person when you need to have a consortium completed. If you have any questions please see her.

There are two different consortiums.  Please fill out the correct one.  You will decide on the correct one by the other institution you are attending.

     If the other college you are attending is a MNSCU college, then complete this consortium.  (there is a list of MNSCU colleges attached to this consortium if you are not sure)
     If the other college you are attending is not a MNSCU college, then complete this consortium.
