Here are a few other Financial Aid forms you might want to complete. 
 

Financial Aid Information Form (FAIF)
To be completed by all students applying for financial aid.

Professional Judgement Form
This form allows special situations to be considered in regards to change in income, unusually large paid medical and/or dental expense, along with other possibilities.

Consortium Agreement Form
May be used to have credits taken at another school during concurrent enrollment be included in aid calculation.

Post-Secondary Child Care Application
Limited funding from Minnesota state government to assist in paying child care expenses.

Loan Request Form
If you already have a loan for 2006-07, this form can be used to ask for additional funding if you are eligible.  Ask the loan officer.
 

Max-Time Frame Appeal Form
May need to be completed if you have been enrolled for more than three years of education at CLC.
