2008-09 Financial Aid Forms

µ   When CLC receives your FAFSA (Free Application for Federal Student Aid) record a "Financial Aid Status"
      report will be mailed to you.  It is also available online at Student E-Services.  Use the report to assist in determining
      which forms to submit to complete your financial aid file.
      Please Note:  For your privacy and protection, it is the policy of CLC's Financial Aid Office to shred any documentation
      received in our office that is not required for your file at that time.

Below is a list of forms used by the financial aid office.  Some of the forms may be necessary for you to complete your CLC financial aid file.  Others may be used to apply for other aid or special circumstances.  Forms in a PDF format need to be printed in Adobe Acrobat Reader.  If you don't have Adobe, click here and choose "Get Adobe Reader".

Internet Explorer is the recommended web browser.
Other browsers may not allow you to complete the forms as required.

     May need to be completed if you have been enrolled for more than

     Limited funding from Minnesota state government to assist in paying child

     Special Circumstances Appeal Form
     This form allows special situations to be considered with regard to changes
     in income, large medical and/or dental expenses that have been paid out of
     pocket, along with other possibilities.		   Spec Circ. Appeal

     May be used to have credits being taken at another school during
     concurrent enrollment be included in aid calculation.

     Loan Request Form (LRF)
     If you already have a loan for 2008-09 at CLC, this form can be used to
     ask for additional funding if you are eligible.  Ask the loan officer.
